How can I reach you between sessions?
If you need to contact me between sessions, please call 224-544-9565 and leave a voicemail and I'll get back to you within a few hours. Another quick way to reach me is via email: akiseloff@bonvietherapy.com. You can also use Secure Messaging in the Client Portal.
Please note that Face-to-Face sessions are highly preferable to phone sessions. However, in the event that you are out of town, sick or need additional support, phone sessions are available. If a true emergency situation arises, please call 911 or any local emergency room.

What is the cost of treatment?
If you elect to pay out-of-pocket fees for services rendered, the rate is $155 per initial evaluation session and $135 per counseling session thereafter. Initial assessment session is usually up to 90 minutes in length. All counseling sessions are 53+ minutes in length unless otherwise indicated.
You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

What types of insurance do you take and what should I know about my coverage?
We are currently accepting insurance from the following providers: Aetna, Blue Cross Blue Shield, Optum/United Healthcare, Cigna, Lyra. We are always working on expanding network of insurance providers, so please contact us for latest information
If you are using insurance, we will submit claims for you. You are responsible for any applicable deductibles, copays and co-insurance payments at the beginning of each session. You should understand that insurance is billed as a courtesy to you and that you are responsible for full payment if the insurance company denies the claim. The rates of health insurance providers vary and maybe different from private pay rates depending on IN-NETWORK contract rates at the time of service. We check your coverage before services are rendered. This information is based on the data provided to us by your insurance provider at the time of the request. You should always verify your benefits with your insurance provider to get the most accurate information on your specific coverage.

What if I need to cancel my appointment?
Please remember to cancel or reschedule 24 hours in advance. You will be responsible for 50 percent of the fee if cancellation is less than 24 hours. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for a session, you may lose some of that session time. The standard meeting time for psychotherapy is 50 minutes. Should you fail to schedule/attend an appointment for 3 consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.

Telehealth and Travelling
We are currently licensed in OHIO and ILLINOIS. You should be aware that state licensing boards require all Telehealth clients to be physically located in the state where their therapist is licensed. You should notify your therapist ahead of time if you plan to leave your state and would like to continue therapy sessions while you are away, so that we can attempt to make arrangements with another state’s licensing board to provide services in that state. You should also know that some state licensing boards may not grant your therapist permission to conduct therapy while you are away in another state.
